You will be asked to fill out a simple registration form for the initial session. It will be helpful if you could arrive several minutes early to your initial appointment to fill out the form in the waiting room; or you can use the online registration form and bring it to the initial session.
We will check your benefits, file your claims, have the insurance company send the funds directly to us so that you are never stuck waiting for reimbursements. Our billing manager will be happy to look up your benefits and provide you with information regarding your out of network coverage.
If you would like the billing manager to look up your benefits, please call her at 888-316-0279 extension 2 and verbally give your insurance details (your full name, name of insurance, provider help line number, your ID, Group Number, Date of Birth) or email the above information to: firstname.lastname@example.org.
Please note that we are not an ‘in-network provider’ even though we do take insurance and work with companies such as Aetna, Cigna, UnitedHealth, Blue Cross, and others. We check your benefits, file your claims, and collect reimbursements on your behalf. Because we are not in-network providers, we are able to maintain a greater level of confidentiality with your medical records. In-network providers are required to share more information with your insurance company; we are free to keep your notes completely private.
